What to Expect During your Visit
Preparing for Your Visit
Please print and complete the Screening Questionnaire. Be as thorough as possible and answer all of the questions. Your immunization history and travel plans are important in determining your travel needs.
In addition to the Screening Questionnaire, please bring the following (if possible):
- your World Health Organization International Certificate of Vaccination (WHO Yellow Book)
- your childhood immunization record
- documentation of all your immunizations
- your travel itinerary with departure dates and all destinations
