About Your Visit
Northwestern Medical Faculty Foundation (NMFF) is located in a state-of-the-art Ambulatory Care Center just east of Chicago's Magnificent Mile. Our Center, housed in the Northwestern Memorial Hospital complex, is located at 675 N. St. Clair Street and occupies 7 floors of the 22 story Galter Pavilion, which is dedicated solely to outpatient care.
To help ensure that your visit goes as smoothly as possible, please take a few minutes to gather the following items to bring with you on your visit:
- Name and address of your referring physician
- List of any medicines that you are or have been taking (including over-the-counter medicines)
- List of any known food or drug allergies
- Important medical information such as medical records, or tests results (X-rays, or laboratory tests)
- Social security number
- Your health insurance card and any required authorization or referral forms
- Financial resources to cover your co-payment or balance due at the time of
service (cash, personal check, Visa, MasterCard, American Express and Discover are acceptable methods of payment)
If you should require inpatient care, all of our physicians are on the medical staff of Northwestern Memorial Hospital, conveniently located adjacent to the Galter Pavilion. In addition, many outpatient tests and procedures are performed at the hospital. |
